Cost of Application Rs.500/- by a demand draft drawn in the name of the Managing Director, Chennai School of Ship Management, payable at Chennai.
Testimonials

Attested copies of following certificates should Be enclosed along with the application form:

1) 10th Std. Marksheet
2) 12th Std. Marksheet (HSC/Intermediate/PUC or equivalent)
3) Age Proof

Medical Certificates

Following medical reports should be enclosed along with application form:

1) Eye Test report (it should include Power of eye
and regarding Colour Blindness)
2) X-Ray Report (X-Ray film can be produced
at the time of personal interview)
3) ECG Report
4) Blood Test Report (as required in the Medical
Examination Report)
5) Urine Analysis Report
